MPT Q3 2026: Debt Refinancing at 9.25% Drives NFFO Decline Even as Revenue Grows 5.3% and Net Income Swings to $202M
Medical Properties Trust posted a sharp swing to $202 million net income in Q3 2026 from a $77.7 million loss in Q3 2025, driven entirely by asset sale gains and a $70 million debt refinancing gain — not by operating improvement. Normalized FFO fell to $63.8 million ($0.11/share) from $77.2 million ($0.13/share) a year ago, as the August 2026 issuance of $2.4 billion in 9.25% secured notes materially increased interest expense, with the weighted-average rate rising to 6.1% from 5.4%. With $1.2 billion of debt maturing within twelve months and cash of $0.6 billion, MPT's capital structure remains the central investment thesis — not operator portfolio performance.

We are a self-advised REIT formed in 2003 to acquire and develop net-leased healthcare facilities. At December 31, 2025, we had investments in 384 facilities and approximately 39,000 licensed beds in 31 states in the U.S., seven countries in Europe, and Colombia in South America. We have operated as a REIT since April 6, 2004, and accordingly, elected REIT status upon the filing of our calendar year 2004 federal income tax return. Medical Properties Trust, Inc. was incorporated under Maryland law on August 27, 2003, and MPT Operating Partnership, L.P. was formed under Delaware law on September 10, 2003. We conduct substantially all of our business through MPT Operating Partnership, L.P. Our primary business strategy is to acquire and develop healthcare facilities and lease the facilities to healthcare operating companies under long-term net leases, which require the tenant to bear most of the costs associated with the property. The majority of our leased assets are owned 100%; however, we do own some leased assets through joint ventures with other partners that share our view that healthcare facilities are part of the infrastructure of any community, which we refer to as investments in unconsolidated real estate joint ventures. We also make mortgage loans to healthcare operators collateralized by their real estate assets. In addition, we may make loans to certain of our operators through our taxable REIT subsidiaries, the proceeds of which are typically used for working capital and other purposes. From time-to-time, we may make noncontrolling investments in our tenants, which we refer to as investments in unconsolidated operating entities. Our business model facilitates acquisitions and recapitalizations, and allows operators of healthcare facilities to serve their communities by unlocking the value of their real estate assets to fund facility improvements, technology upgrades, and other investments in operations.

Revenue commentary · FY 2025
Total revenues declined from $995.5 million in 2024 to $972.0 million in 2025, driven by lower income from financing leases and reduced interest and other income, partially offset by higher billed rent.
